The following forms are mandatory to play lacrosse.  Please note:  Player will sit out at practice until forms are handed in. 

Even if you have completed paperwork for another sport, you are still required to fill out the Information Sheet, and Victory with Honors Forms for both parents and players.  Please note on your Contact Form if your physical and consent is on file for another sport.

Nease Athletic Usage Fee - $50 fee waived if the student-athlete’s family belongs to the Golden Panther Booster Club.

Drivers of Volunteer Vehicles - Required if you plan on driving players in private vehicles.  Also required - copy of driver's license and insurance card. 

If you transferred from another school, you must fill out a transfer form located in the athletic department.
